Home From Hospital Care is supported by the Randal Charitable Foundation

Home from Hospital Care works to optimise the recovery of people returning from health treatment by working to improve discharge planning and aftercare support.

The charity’s voluntary Welcome Home Service is informed of patients needing help from nurses, therapists, social workers and care workers. They then work with specialists in hospitals and in the community, including: general practitioners, carers or patients themselves.

In 2019 and 2021 the Randal Charitable Foundation awarded a grant to assist elderly and people who need support after leaving hospital or require assistance at home.

The Charity was established by its Director, the late Geraldine Amos MBE, in 1972 when it was called The Continuing Care Project.

All volunteers have a range of different qualifications and undertake tasks according to their abilities and location. References and Criminal Records checks are sought for all volunteers.

They emphasise the importance of respecting confidentiality and of behaving sensitively towards patients and carers and regularly hold training sessions covering relevant topics such first aid, bereavement counselling and claiming benefits.

The charity currently offers a service through the four acute hospitals and other health care facilities, to the whole of Birmingham.
